I use Constant Contact, see http://ccprod.roving.com. It has the functionality you say you need, is simple to use yet can be customised straightforwardly. It charges according to how many subscribers you have with it; upfront costs are minimal, and you can easily and cheaply suck it and see.

There are many systems available but I think to enable all the facets you require, a system will need to be tweaked to suit you.
We have a system that does most of what you require and would be suitable to include the extra features. This system has already been taken up by a government organisation so if you would like a first look email and he will be able to take you through the system.

There is a UK company called Simera based in bracknell, they have a good product called
eZ-touch which will do all the things you are looking for the Md is called Nigel Follet - give him a call
