www.surveyshack.com is a new entrant.
If I was making a choice of provider I would look into actual technology they use - one email survey is not the same as the other due to different response rates affected by technological choices. Back in Jungle.com we were running surveys for around 3 years and we had excellent response rates - 70-90% without any incentive primarily due to integrating surveys (or polls we called them) into HTML mail. Most other survey companies would invite people to click on a link to go to a website where survey is located, however, this drastically affects response rate which forces many people to offer incentives, which in my opinion diminishes quality of the survey.
It is my understanding that SurveyShack.com is using embedded in HTML surveys.

disclaimer: I used to work for CEO of that company who at that time was MD of Jungle.com, naturally, he comes highly recommended as under his leadership Jungle became #2 online brand after Amazon. I've given some free advice to SurveyShack a while ago but other than that we are not related.

In case you're interested, after looking at various tools and services (including those mentioned in the replies above) we decided to build our own survey in-house. It's at http://www.e-consultancy.com/other/ratecardsurvey/
In the end the charging models and flexibility of what was on offer meant it made more sense for us to do it ourselves. From what we looked at these tools made most sense for companies conducting a larger number of regular, relatively simple surveys, where there was no in-house expertise available to deliver them. Customer satisfaction surveys, staff surveys and the like seem obvious candidates.

Online Surveys / Customer Research seems to be an area set for quite some growth this year in terms of demand and the number of suppliers (particuarly is it can be 100% web-based making it a global marketplace).

As a customer of Zoomerang for 2 1/2 years, I strongly advise that you do not use their service. My company designed custom import procedures to get our survey results into our CRM and Zoomerang kept making little changes that would mess up the import problems.
I had many months when I could not even use their service due to the fact that they were having a technical problem and it took on average, a few months for them to fix even the slightest problems. Zoomerang may be an inexpensive solution, but the old adage "you get what you pay for" definitely applies in this instance.

I don't understand why people use Zoomerang with all options that are out there. Zoomerang's subscription plans are not flexible at all. Not everyone wants year subscription for 600 bucks!
I use http://www.stellarsurvey.com I pay $10 bucks a month, I get to build any type of survey I want and collect as many results I want. I conducted several surveys with StellarSurvey, so far I been happy with their service.

Globalpark (http://www.globalpark.de/eng/) is a another leading tool for online surveys and online panels. The product is used by a number of corporates like TNS, GfK, DaimlerChrysler ...
