The purpose of the change log is to allow the Project Manager or change team to monitor the number of change requests incoming, and to manage the resources allocated to carrying out impact assessments according to the priority status of the change. It also serves as a complete document of the number of change requests that occur within the project, as well as whether it has been decided to implement them. This is a useful piece of information for the Post Implementation Review, highlighting the efficiency (or otherwise) of the team in defining requirements up front. Once again, this spreadsheet contains some harmless macros to assist with filling in the form.
